We welcome you and your child to Iman Academy Child Care Center SE, where we are
committed to meet your child’s educational and developmental needs. We are a licensed
childcare facility and have been in operation for over 30 years.
To complete the Admissions process, the following list of items needs to be submitted
for your child’s file, at least a week prior to your child’s first day at Iman Academy Childcare.
STEP ONE
1) Admission paperwork needs to be downloaded by right clicking this link then click save link as and select your local Desktop or Download folder, please make sure you have adobe reader installed on your computer. You may want it to be your default PDF APP to open it from your Desktop or Download folder. Once opened you can fill out the form and then print it or save it and email it to us.
2) Copy of Birth certificate
3) Copy of Immunization Record
4) A letter of Good Health from a Doctor.
5) Statement confirming receipt of Iman Academy Childcare Handbook, Click this link

STEP TWO
Admission is completed upon approval by the Childcare Admissions office.
